For HS Applicants

New Students

  • First Year
  1. Need to pass the entrance test
  2. Card or Form 138 with Principal’s signature
  3. Certificate of good moral behavior
  4. Birth certificate
  • Transferees
  1. No failing grades or dropped subjects
  2. Need to pass the entrance test
  3. Card or Form 138 with Principal’s signature
  4. Certificate of good moral behavior issued by the Principal from his/her previous school
  5. Will be accepted under certain terms of conditions or by signing the probationary form
  6. Birth certificate

Old Students

  1. Need to pass all their subjects
  2. Form 138-A
  3. For student with foreign family names they need to bring their birth certificate and/or Alien Certificate of Registration
  4. Certificate of good moral behavior

For Special Science Class only

  1. Weighted Average Grade of 85.00 or higher
  2. No grades lower than 80.00 in any subject except in Math, Science, and English (85.00 or better)
  3. Photo copy of Report Card – F138

Child Study Center

  1. Submit birth and baptismal certificate.
  2. Submit medical certificate.
  3. Pass the entrance exam.
  4. Pass the interview.
  5. Be 4 years old by June of the incoming school year for Kindergarten, and 5 years old for Preparatory

Elementary School

  1. Submit Birth Certificate and Medical Certificate
  2. Submit report card (F138) duly signed by the Principal of the school last attended.
  3. Submit Good Moral Certificate
  4. Complete Kinder and/or Preparatory (for Grade I applicants).
  5. Be 6 to 7 years old in Grade I, and proportionately older in the succeeding grades.
